What we can Continue

Continuation of esketamine (Spravato®) in our REMS-certified program.

Continuation of ketamine: intranasal treatment and/or IV (intravenous treatment available 12/2025).
​How to Continue

Please ask your provider to contact us for a referral. They can call or do so through our HIPAA compliant "For Healthcare Providers" page. Please be sure to include all documents on our Referral Checklist below.

Complete any separate forms we send to you (we will alert you if any are necessary). Please note that standard email is not HIPAA compliant, so it is important to know there is a risk of release of private health information with personal email.

After we review the records, we will reach out, and if it seems like we can help, come in for a brief medical/psychiatric exam.

If it is safe to go forward, begin treatments.
Referral Checklist
Please ask your provider to provide the following (letterhead or EMR printout is fine):

Diagnosis & reason for treatment.

Current ketamine regimen: route, dose, frequency, start date, response/tolerability.

Recent vitals and med list (esp. BP agents, benzos, stimulants, MAOIs).
-
Recent labs/EKG (if available or indicated).
-
Contraindications that have been ruled out.
-
Primary contact info for coordination and urgent issues.
-
Patient releases (HIPAA ROI to share info both ways).

What to Expect at our Clinic

Physician-led, psychiatric care for treatment.
-
Safety-first protocols: screening, monitoring, and outcome tracking.

Comfortable, quiet treatment rooms with continuous observation.

Please plan to be at our office for 1-2.5 hours depending on the method of treatment (nasal versus IV). We do not provide take home medication.

We communicate with your home prescriber regularly.
Exceptions
There are some patients who unfortunately may not do well with this option. Here are possible roadblocks.
-
Non-psychiatric indications. We do not treat pain, performance enhancement, or non-psychiatric uses.
-
Pregnancy or breastfeeding.
-
Normal contraindications to ketamine or esketamine treatments as found elsewhere on this site (generally conditions or situations in which sudden increases in blood pressure would be dangerous).

Active substance intoxication/misuse.
-
Acute need for a higher level of care.
-
Current mania or acute psychosis.
-
Lack of adult escort/rides for the day of treatment.
-
“One-off” or walk-in infusions. We only provide continuation care through active participation with your home prescriber.
Travel notes
-
Airports: Indianapolis International (IND) ~35–40 min from Zionsville (traffic-dependent).

Where to stay: Hotels in Zionsville/Carmel/NW Indianapolis are closest.

Transportation: Ride home with a trusted adult after sessions; no driving or operating machinery until the next day.
Insurance & payment

Spravato®: We will help with prior approval/authorization for the cost of your medication if needed. Medicine will be delivered to the office from a local REMS-certified pharmacy.

All other services are self-pay; we provide superbills for out-of-network reimbursement when applicable.

We’ll confirm costs before your first visit.

Can I start treatment here if I haven’t had it before?

We focus on continuation for visitors. New starts may be possible with a full evaluation—ask our team.

Will my dose be the same as back home?

We try to match your effective dose but sometimes may need to consider adjustments for safety and monitoring per our protocol.

How soon can you see me?

Once we receive a complete referral, we should be able to schedule quickly (subject to availability).

Benefits verification and shipping or handling of your medication may require 2 to 3 business days.

Final treatment eligibility is determined after records and exam review.

Please be certain you have a responsible adult to take you home on the date of treatment, and please do not drive, travel, or make important life decisions for the rest of the day after treatment.

We are not an emergency facility. Please go directly to the emergency department or call 911 if you are in a crisis situation.
